Aged Care Support is able to provide advice, support and assistance to older people and their families with the set up and ongoing care in the home or aged care facility, within the Wellington region. Our services range from companionship through to high level care.
Sometimes family and friends do not know where to start and having the advice of someone who can provide support through the process can make a real difference to the experience of your family member.
By having an experienced Registered Nurse guide you through this process, we will together be able to provide the best care for your elderly family member. This could include the initiation of District Health Board (DHB) funded in home care and subsidised prescriptions if you qualify.
This includes assistance with medical care, assistance after a hospital stay or illness, ongoing GP and outpatient appointments and assistance and advice with transition from home to a residential or hospital setting.
A free of charge initial meeting with family can set out a plan of care for your family member in which we’ll determine the level of assistance that will be needed. The service that we provide could be a one off arrangement or an ongoing plan of support.
